Replace a Ghana Card
If your card is lost, stolen, damaged or defaced, report it to the police, then visit a regional or district office for a new one.
What to bring
- Lost or stolen: the original police report or police extract. A photocopy is not accepted.
- Damaged or defaced: the police extract and the damaged card.
What happens at the office
- A registration officer reviews your police extract and, if the card is damaged, the card itself.
- The officer helps you complete and sign or mark the electronic request form, and submits it for you.
- An approval officer reviews the request. If it is declined you are told why.
- Once it is approved, you pay at a CalBank branch, with the CalBank app, or by dialling *771#. A damaged card is taken from you.
- The officer enters the serial number and PIN from your payment, and takes your fingerprints, iris scan, photograph and signature.
- Your new Ghana Card is printed and handed to you.

Questions
Can I ask for a replacement without the request form?
No. Every request must be signed or thumbprinted by the applicant. If you signed your registration form, you sign the request form too.
Is a photocopy of the police extract accepted?
No. Bring the original. The officer scans it into the register and gives it back to you.

The police extract and the damaged card itself.
